Bill Gates talking about killing of 0.9 billion people with health care and vaccines

Can someone explain this to me.

Bill Gates: "First we´ve got population. Now, the world today has 6.8 billion people. That´s headed up to about nine billion. Now, if we do a really great job on new vaccines, health care, reproductive health services, we could lower that by, perhaps, 10 or 15 percent, but there we see an increase of about 1.3."
So is Mr. Gates talking about decrease the world population by 0.9 billion people by changing health care systems and deliver vaccines to people?
Does that make sense to you? What was his point? Did someone ask him what he meant by that? Who is the "we" he´s talking about? And who is among the 900 000 000 that "they" want reduce/remove/kill?
